  • Driving Licence – you must always hold a full valid driving licence  
  • Essential Car Documentation- This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance.   
  • DBS (Disclosure and Barring Service) clearance - this post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed here. We have a policy on the Employment of Ex-Offenders and it is available upon request.   
The Integrated Neighbourhood Teams (INT) consists of established community teams bringing together social care, community health, mental health and primary care and we are looking to bolster these teams with driven, qualified Social Workers.

You will be expected to complete face to face assessments in peoples homes and other residential settings, you will need to complete Care Act Assessments, support plans and review of new and current care plans. 
You will also need to be familiar with and competent in completing residential placements and safeguarding, while working within a multidisciplinary team.

This  role is part of a large team of 8 social workers and 1 Social Care Assessor and you will be required to support duty working on a rota basis. 
You should have full knowledge of policy and legislation including CHC, MCA, BI and DOLs soon to be Liberty protection safeguards. We are looking for someone who is proactive and enthusiastic. 

Some duties include:
  • Generic neighbourhood-focused social work
  • Carrying out Best Interest Assessments (we’ll support you to qualify if you’re not already)
  • Mobilising our new place-based home care model
  • Undertaking reviews and reassessments to ensure we are fulfilling our Care Act requirements.

About the Candidate

  • Develop and manage a relationship to support service users and their families in line with statutory obligations through complex decision making to allow service users to access improved future opportunities. 
  • In conjunction with partner agencies and stakeholders, effectively assess and plan service users’ individual needs to ensure positive outcomes for Manchester residents. 
  • Develop positive and effective solutions in all aspects of service delivery and engagement, focusing on the quality of outcomes for service users.  
  • Collaborate with internal and external colleagues and stakeholders to actively contribute to the development and delivery of the service within Manchester City Council and partner organisations. 
  • Engage with stakeholders in the private and public sectors and across local communities to encourage a collaborative and transparent approach that promotes service users’ needs across a range of existing and new initiatives. 
  • Efficiently maintain accurate and appropriate records in accordance with statutory targets and City Council, local and national policy and procedures to evidence achievement of service targets and improvements in delivery. 
  • Through personal example, open commitment and clear action, ensure diversity is positively valued, resulting in equal access and treatment in employment, service delivery and communications 
Roles at this level may be required to manage a range of assigned resources, which may be human, financial or other, to ensure continuous improvement in service delivery.  Staff management duties may be either through direct line management of a team (including appraisals, performance management and other duties) or through matrix management of a virtual team of officers.
