Adult Social Work
Social workers carry out Care Act 2014 assessments. We work in a very strengths-based, holistic way, alongside partners. Our teams include:
- Integrated Neighbourhood Teams working across the twelve Manchester neighbourhoods work closely with GPs, nurses, health development coordinators and mental health, under the direction of a Neighbourhood Lead.
- Integrated Learning Disability – a specialist team working alongside health professionals in a holistic way.
- Hospital Discharge – based in hospitals sand also doing assessments in the community as part of discharge arrangements.
We also have lots of specialist teams with opportunities to move between fields including the Multi-Agency Safeguarding Hub, substance misuse, citywide care homes, transition to adulthood, sensory, review and case management.

Reablement
Helps people regain or attain as much independence as they can in their own homes. Support workers motivate and inspire people to re-learn daily skills and enhance their quality of life.
Disability Supported Accommodation and Short Breaks Service
Supports those with learning and physical disabilities to live in their own home and as independently as possible, supporting choice and developing skills. The Short Breaks service helps as part of respite.
Equipment and Adaptations, and Community Alarm
Provides equipment and minor and major adaptations, following assessment, to those needing support for independent living in their own environment. The Community Alarm service monitors technology in people’s homes that helps them remain independent for longer, helping those who’ve had a fall at home.
Day Services
Varied and flexible services for adults with a learning or physical disability, older people, adults with an acquired brain injury and people with sensory impairments. The service aims to make a positive difference by promoting independence, health, wellbeing and choice in a person-centred way.
