This role is subject to the following requirements:
- Driving Licence: You must hold a full, valid UK driving licence at all times.
- Essential Car Documentation: This post is designated as an Essential Car User. You must provide your own vehicle for work purposes and will receive the Essential Car User Allowance.
- DBS Clearance: This post requires an Enhanced Disclosure and Barring Service (DBS) check, in line with the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended 2013). All information will be handled confidentially and in accordance with the DBS Code of Practice.
- Employment History & References: You must provide a full employment history and evidence of conduct in previous roles involving health, social care, or work with children or vulnerable adults.
- Proof of Identity: Including a recent photograph.
You will report to the Registered Manager of Shared Lives and the Senior Shared Lives Placement Worker. Key contacts include Shared Lives and Supported Lodgings Providers, vulnerable adults, families, carers, health professionals, members of the public, and other Manchester City Council departments.
Key Role Accountabilities
- Build and maintain trusting relationships with citizens, supporting them to live independently and safely within Shared Lives arrangements.
- Conduct thorough assessments of both carers and individuals, ensuring compatibility and safeguarding needs are met.
- Develop and review person-centred support plans that promote independence, wellbeing, and inclusion.
Lead in the quality assurance and review of individual citizen’s files ensuring they meet the standards set by CQC and the Services Quality Assurance Framework
- Provide ongoing supervision, guidance, and support to Shared Lives carers, ensuring high standards of care and compliance with regulatory frameworks.
- Collaborate with internal teams and external agencies to coordinate holistic support and ensure positive outcomes for individuals.
- Promote the Shared Lives model across the organisation and with external partners, contributing to service development and innovation.
- Maintain accurate and timely records to support performance monitoring, quality assurance, and regulatory compliance.
- Participate in recruitment, training, and approval processes for new Shared Lives providers.
- Support service users through transitions, including emergency placements, respite care, and long-term arrangements.
- Uphold the values of dignity, respect, and equality in all interactions and service delivery.
- Contribute to team development and may be required to undertake supervisory responsibilities.
