Medical Clearance - Pre-placement health assessment (PPHA)  

Essential Car Documentation- This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance. 

DBS (Disclosure and Barring Service) clearance - this post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed here. The social care assessor role will be in the Citywide Care Homes Team. This role involves arranging timely support assessments, equipment and minor adaptations to support independent living, assistive technology and support carers and family members.

Advising citizens and referring them to appropriate services based on the observation and assessment completed, ensuring that the provision of services supports citizens maximise their independence.

Developing and implementing citizen support plans to meet identified social needs of individuals, enabling the citizen to achieve maximum health and well-being.

Establishing and maintaining relationships with citizens and working proactively with appropriate stakeholders to assess the individual’s needs and support positive outcomes for citizens. Complete strengths based holistic assessments and observations, identifying and arranging appropriate adults’ services to meet the needs of the citizen.

Providing and facilitating the provision of intervention from other agencies or departments. Identifying safeguarding and mental capacity issues and referrals as appropriate; assisting in investigations of allegations of any form of abuse to vulnerable adults

Actively participating in new initiatives and future changes intended to improve service delivery. Maintaining and participating in duty function Multi-disciplinary working


About the Candidate

We are looking for an experienced candidate who can demonstrate the following skills:

Communication: Able to demonstrate an understanding of the views of others, who communicates in a realistic and practical manner using appropriate language and medium and listens attentively to views and issues of others and responds to issues arising.

Good Analytical Skills: Ability to absorb, understand and quickly assimilate complex information and compare information from a number of different sources.

Problem Solving: Ability to problem solve, identify suitable solutions and  effective decision making. Able and confident to resolve moderately complicated queries in their area of knowledge using logical thinking to explain reasoning behind decisions or actions taken.

IT Skills: Ability to use ICT systems to obtain and analyse data and present it effectively through a variety of ICT channels
