Essential Car Documentation - This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance. 

DBS (Disclosure and Barring Service) Clearance - This post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed here. This is an exciting opportunity to join the Building Independence Team within Reablement. This new model of care for specialised Reablement will ensure a dedicated team of trained and skilled support workers, competent in supporting individuals with specialised needs to make lifelong sustainable changes to achieve their goals.

 
The Building Independence Team will offer a more efficient and consistent approach to support which will be a significant step forward in providing comprehensive, specialised support to a diverse range of individuals. This approach not only maximises the potential for individuals to achieve reduction in care needs but also ensures consistency and high standards of service delivery.

The new Reablement provision will support the following cohorts while utilising and maximising the TEC offer:

Individuals with substance misuse
•    Individuals with low to medium mental health needs 
•    Individuals with learning disabilities 
•    Individuals with autism
•    Individuals with neurodiversity 
•    Support to residential/nursing provision where 1:1 support is identified, offering approaches to manage distressed            behaviours and falls 
•    Support for individuals with early-stage dementia to remain in their own homes. 
 
The successful candidates will join a service that prides itself in seeing people achieve their goals and return to independence. 

The candidate will work in a community setting citywide within an individual’s own home to support them to achieve independence, longer-term solutions and aspirations whilst maintaining their privacy and dignity. 

The post holder will provide and promote choice and control to the individual which will help them transition to maximum independence through a person-centred approach and partnership working.  

The post holder will maintain accurate, up to date records and collect data and information to evidence performance around service delivery to ensure achievement of internal and external targets

About the Candidate

We are looking for people who are flexible and are able to undertake shift work, as the service operates from 7.30-10pm Monday to Sunday 365 days a year. 
 
The post holder should have the following attributes to offer: 
 
Being able to adapt to various situations and be observant.
Have excellent communication skills
Be confident in what you do.
Proactive to support customers to achieve their identifiable goals.
Have positivity which improves and supports team morale, and which can have a positive influence on their                    surroundings. 
Being resilient to deal with emergency situations and find resolutions to problems
Be able to liaise with a variety of partners and work under pressure
Ideally the post holder should possess some IT skills, but training will be provided.
 
The post holder should be willing to undertake mandatory training to improve personal development and be able to work as part of a team and on their own using their own initiative. The post holder is required to be a car user and holds a full clean driving license. The post holder should be willing to undertake an enhanced DBS check.
