Essential Car Documentation- This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance.   

DBS (Disclosure and Barring Service) clearance - this post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). The Hospital Discharge to Assess Social Work Service in Manchester has a vital role in supporting safe, timely and person-centred discharges from hospital, ensuring that people receive the right support in the right place at the right time. Working at the heart of a busy acute hospital system, the service brings together social care, health colleagues and wider partners to promote independence, prevent unnecessary hospital admissions and reduce delays in discharge.

This is an exciting and impactful opportunity to work in a fast-paced, multi-disciplinary environment where professional judgement, collaboration and strengths-based practice are central. As a Social Worker within the Discharge to Assess pathway, you will play a key role in supporting individuals and their families at critical points in their lives, ensuring that assessments, care planning and decision-making are person-centred, lawful and focused on achieving positive outcomes.

About the Candidate

We are looking for an experienced and confident Social Worker who can demonstrate a strong track record of applying legislation in practice, and who is committed to delivering high-quality, strengths-based support within a fast-paced hospital environment.

This role requires a practitioner who is able to evidence clear competence in interpreting and applying statutory duties. You must be confident in making lawful, defensible decisions in complex situations, ensuring that all practice is rooted in legislation and centred on the person’s rights, wishes and outcomes.

You will be able to balance risk, independence and least restrictive practice while working under pressure, and be confident in challenging within multi-disciplinary environments to ensure safe and legally compliant decision-making.

We would love to hear from you if you have experience of:

  • Applying the Care Act 2014 and Mental Capacity Act 2005 in complex practice situations

  • Making lawful, defensible decisions in high-pressure environments, particularly within hospital discharge pathways

  • Undertaking Mental Capacity Assessments and supporting Best Interests decision-making

  • Managing risk in line with statutory duties, balancing safety with independence and least restrictive options

  • Representing social work within multi-disciplinary teams and advocating for individuals’ rights and outcomes

Essential requirements for the role:

  • A recognised Social Work qualification

  • Registration with Social Work England (or eligibility to register)

  • Demonstrable and consistent experience of applying key legislation in practice, including:

    Care Act 2014

    Mental Capacity Act 2005 (including capacity assessments and Best Interests decision-making)

    Safeguarding duties and relevant statutory guidance

  • The ability to make lawful, defensible decisions in complex and high-risk situations, including where there is uncertainty, system pressure or conflicting professional opinion 

  • Experience of undertaking complex, person-centred assessments and developing appropriate care and support plans

  • Strong understanding of risk assessment and proportionate risk management, including application of least restrictive practice

  • Ability to clearly evidence and justify decision-making through accurate, timely and defensible record-keeping

  • Experience of working autonomously within a high-pressure environment, using professional judgement and accountability

  • Strong communication skills, including the ability to manage complex discussions and challenge appropriately within professional forums

  • Experience of working effectively within multi-disciplinary teams and with partner agencies

  • A clear commitment to strengths-based, person-centred practice and promoting independence

  • Enhanced DBS clearance

  • The experience below would be beneficial but is not essential

  • Experience of working within a Discharge to Assess (D2A) model or hospital discharge service

  • Understanding of “Home First”, reablement and intermediate care pathways

  • Experience of applying legislation within fast-paced hospital systems or time-critical environments

  • Experience of working with individuals with complex needs, including fluctuating capacity and high levels of risk

Desirable requirements that would be useful in this role:

  • Previous experience within a hospital social work or discharge team

  • Experience of working across health and social care systems to support safe and timely discharge

  • Experience of contributing to Best Interests meetings and complex multi-agency decision-making forums

  • Experience of working with a range of partner organisations, including voluntary and independent sector providers

  • Strong ICT skills, including use of case management systems and Microsoft Office.
