The role holder will provide support as part of a short term reablement service which coaches and encourages customers to learn or re-learn daily living skills, enabling them to confidently and independently live in their own homes and take part in the community.

To support the customer to achieve their goals, promote independence, health and well-being throughout the ‘make safe and stabilise’ and/or reablement support period.

Provide person centred, high quality support to customers in their own home or in other appropriate locations, to maintain and develop daily living skills that will enable full participation in a range of activities which support customers to access appropriate community services.

Provide practical, personal and intimate care whilst maintaining high levels of hygiene and health and safety. 

Work with customers in goal setting, as detailed in the support plan to enable the customer to maximise their independence and rehabilitation through reablement programmes and emotional support.  To contribute to future support planning by monitoring, evaluating and communicating with colleagues supporting the customer journey, external partners and stakeholders.

Establish customer needs and think innovatively to deliver solutions utilising local community groups/ resources to support the customer to achieve their goals.

Complete all documentation and correspondence accurately and to a high standard in line with procedures and within agreed timescales, noting progress made and any changes of significance to the health, safety and wellbeing of customers to relevant colleagues and stakeholders.

Work in line with National Care Standards, Care Quality Commission and City Council policies and procedures, incorporating changing legislation and best practice.  Maintain a thorough understanding of safeguarding procedures with the ability to recognise and respond appropriately to any issues that arise and participate in any investigations as appropriate.

Proactively participate in new initiatives and future changes in service delivery.

About the Candidate

Demonstrate a clear understanding of care procedures to provide high quality care and support to service users and assist individuals with accessing and participating in appropriate activities and community services.

Establish service user needs and think innovatively to deliver solutions utilising local resources where available to support the service user in achieving their goals.

Contribute effectively and innovatively with the creation and implementation of assessments, plans and risk management to ensure positive future outcomes for service users.

Provide personal, practical and if required intimate care whilst maintaining high levels of hygiene and health and safety.

Ensure that records are completed and maintained with sufficient information and accuracy in line with agreed service standards and timescales. Where appropriate raise any issues or changes of significance to relevant colleagues and stakeholders.

Personal commitment to continuous self development and service improvement.

Through personal example, open commitment and clear action, ensure diversity is positively valued, resulting in equal access and treatment in employment, service delivery and communications

Where the role holder is disabled every effort will be made to supply all necessary aids, adaptations or equipment to allow them to carry out all the duties of the role. If however, a certain task proves to be unachievable, job redesign will be given full consideration.

 
  • Interpersonal skills: Ability to communicate clearly and effectively taking account of individual needs and ensuring that this information is understood.
  • Planning and Organising: Demonstrate the ability to organize multiple tasks in the most effective way and allocate time and energy according to task complexity and priority.
  • Analytical Skills: Ability to understand moderately complex information and compare information from a number of different sources. Ability to identify patterns and trends that may impact on decisions with skills to identify risks and any assumptions made. 
  • Problem Solving and Decision Making: Ability to interpret rules and guidelines and know when something needs to be referred to a Manager
  • Communication Skills: Demonstrates an understanding of the views of others and communicates in a realistic and practical manner using appropriate language and medium, listens attentively to views and issues of others and responds to issues arising.
  • Administrative Skills: A good level of literacy and numeracy skills to manage the maintenance of customer records.
