Medical Clearance - Pre-placement health assessment (PPHA)

DBS (Disclosure and Barring Service) Clearance - this post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). The Complex Needs and Preparing for Adulthood Service is made up of several multi-disciplinary award-winning citywide teams which sit within Manchester Adult Social Care, supporting vulnerable and multiply excluded citizens develop fulfilled, meaningful and safer lives.  

 

The Service is made up of several, specialist teams which include, Preparing for Adulthood, The Drug and Alcohol Team, Entrenched Rough Sleepers Team, The Sensory Team, The No Recourse to Public Funds Team, The Direct Payments team and The Emergency Duty Service.   We are also developing the Autism and Personalisation team.   

 

Over the last 3 years, significant investment has been provided to the Complex Needs Service, and within each team we have several specialist roles which include, an Autism Lead, Social Workers, Approved Mental Health Professional, Social Care assessors, Person centred planners, Link Workers, Vision Rehabilitation Specialist, Specialist Sensory equipment assessment and installation officers, Direct Payment Coordinators, Assertive Outreach Workers, No Recourse to Public Funds Coordinators, Service Improvement and Development Leads and Project Officer.   

 

Each role profile has a specialist training plan, to ensure staff are skilled and offered continued development.  There are also lots of opportunities for career progression through our apprenticeship progression schemes. 

 

The Complex Need and Preparing for Adult Service is an exciting landscape to work in, which is evidence based and working on the cutting edge of social work.  If you like fast paced, innovation work where you will be given autonomy but high levels of support, this is the Service for you.  This team doesn’t say we can’t…. it says how can we.  

 

The role holder will be based in the Preparation for Adulthood service. We are a citywide team that collaborates with young people and their networks to create a personalised plan that addresses the individual’s specific needs, goals, and aspirations as they move into adulthood.

The service is also referred to as the ‘Transition Service’ used to describe the transfer of care and support for vulnerable young people from Children’s Social Care to Adult’s Social Care. The aim is to ensure a seamless transition, providing necessary support and services during this important life stage.

The Social Care Assessor will be responsible for undertaking holistic Care Act 2014 assessments. The role holder will undertake home visits and carry out holistic strengths-based observations and assessments of citizens to support a determination of social care needs.

 

Key Responsibilities will include:

 

Arranging timely support considering reablement, considering packages of care, short breaks, carer assessments, equipment and minor adaptations to support independent living, assistive technology and support carers and family members.

 

Advising citizens and referring them to appropriate services based on the observation and assessment completed, ensuring that the provision of services supports citizens maximise their independence.

 

Establishing rapport with citizens and working proactively with appropriate stakeholders to assess the individual’s needs and support positive outcomes for citizens.

 

Complete strengths based holistic assessments and observations, identifying and arranging appropriate adult services to meet the needs of the citizen.

 

Identifying safeguarding and mental capacity issues and making referrals as appropriate; assisting in investigations of allegations of any form of abuse to vulnerable adults.

 

Actively participating in new initiatives and future changes intended to improve service delivery.

About the Candidate

We are looking for someone with the following Skills:
 
  • Communication Skills: Demonstrates an understanding of the views of others and communicates in a realistic and practical manner using appropriate language and medium, listens attentively to views and issues of others and responds to issues arising
  • Analytical Skills: Ability to absorb, understand and quickly assimilate complex information and compare information from a number of different sources. Able and confident to resolve moderately complicated queries in their area of knowledge using logical thinking to explain reasoning behind decisions or actions taken.
  • Planning and Organising: Ability to organise own time effectively, creating own work schedules, prioritising, preparing in advance and setting realistic timescales. Has the ability to visualise a sequence of actions needed to achieve a specific goal and how to estimate the resources required. Ability to maintain sound judgement under competing priorities and pressure.
  • IT Skills: Ability to use ICT systems to obtain and analyse data and present it effectively through a variety of ICT channels.
  • Problem Solving and Decision Making: Is able to make effective decisions on a day-to-day basis, taking ownership of decisions, demonstrating sound judgement in escalating issues where necessary. Be logical in their thinking and explain reasoning behind decisions or actions taken
